We are a Patient-Centered Medical Home certified by The National Committee for Quality Assurance (NCQA), and put patients at the forefront of their health and wellness. Studies have shown that PCMHs result in better relationships between patients and their clinical care teams, and improve the overall quality of the patient experience.

Conyers has now grown to 15,000 residents and Rockdale County to over 80,000. It is a combination of small town and big city, of rural scenes and urban shopping. The population now reflects many nationalities, races and religions.
